BURIAL INSTRUCTIONS FOR PRESTON JOSEPH RODDY JR


       I, Preston Joseph Roddy Jr., a resident and domiciliary of Lafourche parish, Louisiana, being of sound mind, willfully and voluntarily make known my
       desire of a burial and do hereby declare:

       An Authorizing Agent will be required by law to proceed with a cremation. I have established an account with SAMART, Bayou Blue, and have
       already signed documents for that purpose. However, someone from my family must also sign as authorizing agent at the time of my death, to sign all
       forms, documents, or other papers with the funeral establishment that are required to authorize the crematory authority to cremate his remains and
       to do and perform any and all acts necessary in accordance with the requirements of law.

       I direct that anyone of my survivor siblings or nephews LYNN PARTRICK RODDY, VERL JEROME RODDY, KEARNEY JOHN RODDY,
       FARON MICHAEL RODDY OR COREY JOHN RODDY, IAN PAUL RODDY may act as authorizing agent.

       SAMART FUNERAL HOME OF BAYOU BLUE will be responsible for my cremation & ashes be placed into a brass URN and prominently displayed
       at the time of a brief memorial service, by a priest from St Joseph Church, Chauvin, Louisiana 70344. The ashes within the URN will be further hand-
       carried by a family member and further transported by private owned vehicle to St. Joseph Cathedral Cemetery, in Thibodaux, La. 70301,
       where the URN with ASHES will be received and sealed into a NICHE, and recorded with the St. Joseph Cathedral Cemetery Administrative office.

       The Niche vault will be sealed with a placard. The Placard will essentially read; Preston Joseph Roddy Jr. - Born 6-8-1952 Died xx-xx-xxxx
       - Army Veteran - 1972-1998. A copy of my DD Form 214, military discharge and a death certificate will be needed to authorize the veterans
       Administration to produce a placard as read above. My service record will demonstratean active Army draft service 1972-1974, an inactive
       Army reserve service 1974-1980 and an active Army re-entry service record 1980-1998.

       Although reporting my death to a Veterans Administration Office is necessary for the purpose of ending all my military benefits,
